Carr was in her forties when she landed her first professional acting job as Mother Courage, followed by another Bertolt Brecht production, The Exception and the Rule, at the Young Vic. Clarissa Mullery, a frequent character on the long-running BBC murder drama series Silent Witness, joined her as a regular character in 2013. After eight years on the show, Carr announced his departure from Silent Witness on February 5, 2020.

Carr played Dr Marlow Rhodes in The OA in 2019. Carr starred as a university lecturer in the Devs miniseries in 2020.

According to the show’s official website, Carr was cast as Fenn in The Witcher’s second season in March 2021. Carr was cast as the angel Saraqael in the second season of Good Omens, which premiered in December of the same year. Carr played Dr Emma Brookner in the West End revival of The Normal Heart at the Royal National Theatre from September to December 2021, marking the first time a disabled person had played the character on stage. At the 2022 Laurence Olivier Awards, Carr won the Laurence Olivier Award for Best Actress in a Supporting Role.
